What is the Supported Living Guidebook?

Supported Living is a service model that helps people with intellectual/developmental disabilities (IDD) live independently in their own homes by offering them support. This guidebook is intended to provide individuals with IDD, natural supports, service providers, and other interested stakeholders the practical information about the Supported Living service model in order to set up a home for life-long success.

This guidebook project was funded by NCCDD and Money Follows the Person, both committed to supporting persons with disabilities to live in their own home.

Additional resources are provided here as supplemental materials for the guidebook. We aim to keep this section updated and continuously add materials to support the effective implementation of supported living models and services.
